Data collection is the first step in our process. This essential first step allows us to gather extremely detailed data from your organization. Using our how2trak® system, information is collected on care practices in your organization. Data is retrieved and analyzed using quality metrics that are based on specific indicators identified by your organization. The result of this analysis is your baseline assessment.
We can compare your baseline assessment against other disease benchmarks using our proprietary bank of chronic disease data. This will allow you to benchmark care/cost data with other comparable organizations to facilitate improvements in care delivery, assess your current human and financial resource allocation and plan strategically for the future
Our innovative approach to outcome measurement using our web-based system enables us to generate population data rather than individual client data and our unique rapid real-time reporting capability distinguishes our outcome measurement solution from other systems. Our approach to outcome measurement allows you to generate custom reports that will immediately identify areas of care/cost success and areas of care/cost improvement.
